The Middle Grade Pharmacist will rotate through a variety of specialities to include surgical, medical, critical care and oncology. This new position requires post-holders to have at least one-year's hospital pharmacy experience as well as a willingness to commence their post-graduate diploma if not already undergoing this. This post will have limited dispensary requirements (max 4hrs per week), close support from the Head of Pharmacy and all Senior Pharmacists is provided.
Job Responsibility:
Provide clinical and professional leadership to the pharmacy team and medicines management service
Identify and promote best practice
Motivate and inspire others
Proactively develop clinical pharmacy services in line with local and national objectives
Work collaboratively with pharmacy, medical staff, the multidisciplinary team and management
Negotiate with and inform clinicians on prescribing practice issues
Promote best practice prescribing through co-ordination of projects and audits
Ensure safe, clinically effective and cost effective use of medicines
Contribute to monitoring and management of drug expenditure
Identify changes in prescribing practices impacting drugs budgets
Identify new prescribing practices impacting the drugs budget
Be available to answer queries relating to individual patients and policies
Attend the Drugs and Therapeutics committee as required
Undertake audit and investigations and implement changes
Present results of audit work at National Meetings if appropriate
Develop, implement, monitor and evaluate protocols and guidelines
Update protocols and guidelines on a rolling basis
Participate in pharmaceutical aspects of clinical audit
Liaise with clinicians in smoothing entry of new drug protocols and phasing out old practice
Provide a high quality, customer focused pharmaceutical service
Review prescriptions for clinical appropriateness, safety and legality and conduct medicine reconciliations
Dispense and supply drugs to patients as per service demand
Provide evaluated pharmaceutical advice and information using online databases and primary sources
Have a working knowledge of pharmacy stock control and ordering systems
Act as a clinical role model
Professionally screen/check prescriptions
Integrate research evidence into practice
Actively seek to improve ward and clinical services
Demonstrate expert clinical knowledge
Plan, monitor and review drug treatment protocols
Ensure medication histories and patients' own drugs are checked on admission or transfer

Work with Infectious Diseases/Microbiology consultants to develop best practice protocols
