Tameside Metropolitan Borough Council is seeking an experienced and passionate Mental Health Transitions Social Worker to join their specialist team supporting young people as they move from Children's Services into Adult Services. The role involves working with young people experiencing moderate to severe mental health difficulties, including those with autism and dual diagnoses, ensuring they receive the right support at a critical stage in their lives. The post holder will play a key role in promoting independence, planning for adulthood, and helping young people and their families navigate complex transitions.
Job Responsibility
Supporting young people transitioning from Children's Services into Adult Services
Undertaking Care Act assessments and developing clear, outcome-focused support plans
Completing funding applications and statutory documentation
Managing complex legal work including Court of Protection DoLS applications, DoLS, Section 21A challenges, and court reports
Building meaningful engagement with young people who may be difficult to engage and supporting their families and carers
Undertaking Carer's Assessments and supporting carers under the Care Act
Working closely with SEND services and understanding the role of Education, Health and Care Plans (EHCPs)
Promoting strengths-based practice and person-centred planning
Working collaboratively across health, education, children's services, and adult social care to ensure seamless transitions
Contributing to a responsive six-day service, primarily Monday to Friday with occasional Saturday working
Requirements
Social Work Degree, DipSW or equivalent professional Social Work qualification
Completion of ASYE
Current registration with Social Work England
Full UK driving licence and access to a vehicle for work purposes
Minimum of 2 years' post-qualification experience
Substantial experience working with young people transitioning from Children's Services into Adult Services
Significant experience working within Mental Health Services
Knowledge and experience of SEND and Education, Health and Care Plans (EHCPs)
Experience of Care Act assessments, care planning, safeguarding, and statutory social work practice
