Job Description

New Start has an exciting opportunity for a Mental Health Support Practitioner to join our Adults Services Team based in Liverpool. This is a rewarding role where you will make a real difference, supporting vulnerable adults to achieve positive outcomes and lead fulfilling, independent lives.

Job Responsibility

  • Provide both practical and emotional support to individuals with mental health needs, helping them work towards greater independence and improved wellbeing
  • Support residents within our services, as well as visiting vulnerable adults in their own homes to deliver outreach support
  • Help individuals maintain their independence, develop life skills, and manage their tenancy and daily living responsibilities effectively
  • Implement and facilitate activities that promote personal development across areas such as education, social inclusion, emotional resilience, physical health, and positive mental wellbeing
  • Support individuals to manage medication, sustain engagement, and develop independence
  • Assist with the safe management of buildings, respond appropriately to incidents, and ensure all reporting and recording requirements are met

Requirements

  • Previous experience supporting clients with mental health issues
  • Excellent communication skills with a calm, patient, and confident manner
  • Experience supporting vulnerable individuals through crisis or periods of change
  • Experience assessing the needs of vulnerable adults, particularly those at risk of homelessness - Desirable
  • Experience of working collaboratively with other agencies (voluntary and statutory)
  • Experience of casework, support planning, risk assessments, and key working
  • Full driving licence, access to your own vehicle, and business insurance
