We are recruiting experienced Support Workers to join a service supporting adults (18+) with mental health needs, substance use, and alcohol-related challenges. This role involves managing challenging behaviours, though no physical risk is expected.

  • Actively manage the shared BH registry to track individual patient progress
  • Prepare and attend weekly registry review sessions with Psychiatric consult

  • Develop care plans and facilitate referrals to external resources
  • Provide short-term counseling and evidence-based treatments within the Collaborative Care Model
