An exciting opportunity has arisen for a band 5 Nurse Practitioner to join our acute inpatient team. You will provide a high level of care and support to people with complex care needs on long term placements. The post holder will work closely with multi-disciplinary teams to ensure suitable packages of care for service users. The post holder will work with the team to provide a person-centred approach is delivered.
Job Responsibility:
To understand and follow good practice within the framework for the Mental Health Act, Mental Capacity Act and DoLs and Care Programme Approach
To maintain accurate records, both written and computerised to a standard that reflects their level of clinical expertise
Liaise with community based services and resources such as Focus, housing, Police and voluntary sector organisations to facilitate the delivery of the most appropriate and best possible care
Discuss issues surrounding risk management and plan care accordingly advising others regarding management of care with senior practitioners
Discharge planning
To respond to service user crisis in a positive pro-active manner
To follow clear care pathways, including methods to reduce the risk of further crisis developing
To work flexibly within a 24 hour rota covering early, late and night shifts according to service user need
To provide telephone support to any service users, carers, outside agencies or clinicians who require information, advice or crisis intervention
Undertake comprehensive CPA mental health assessments then develop and implement care packages in a variety of settings
To assist in maintaining safe custody of drugs in accordance with the Trust policies and procedures
To cover 7 days a week, 24 hours a day on a rota
Requirements:
RMN or awaiting Registration
Evidence of current registration
Basic IT Skills
Able to respond to service users, carers and significant others in a person centred way
Able to maintain a safe environment for service users, carers, staff and others
Able to work as a team member and effectively communicate with service users and other professionals
Able to assess individuals. Formulate, implement and evaluate care plans
Able to participate in clinical supervision
Demonstrate and understanding of the Mental Health Act, Care Programme Approach, Mental Capacity Act and DoLs
Experience of working with people with mental health problems
Experience of working with older adults with mental health problems
Nice to have:
Evidence of ongoing professional development since qualification
