The role is integral in ensuring that Cumbria, Northumberland, Tyne and Wear NHSFT and Tees, Esk and Wear Valleys NHSFT as lead providers meet their accountabilities and responsibilities in terms of commissioning for quality. To provide clinical assurance to the PC. As a Case Manager, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users from their Provider Collaborative (PC) in specialised mental health care settings, within the NHS and Independent sectors.
Job Responsibility:
Case Management of specialised commissioned within the Provider Collaborative within the defined population
Provide clinical assurance and quality oversight for the defined population of commissioned services
Managing individual patient pathways to ensure quality services and care planning outcomes are achieved
Work and engage with internal and external stakeholders on a range of sensitive issues
Nurture key relationships and maintain networks internally and externally, including national networks
To provide links with Integrated Care Systems (ICS) and their commissioned services within the PC to ensure there is a co-ordinated approach to care and discharge planning
Requirements:
Registered Mental Health or Learning Disability Nurse or registered health/social care professional
Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master's level equivalent
Detailed knowledge of legal issues pertaining to the specialist service, including MHA 1983, after care under supervision, sex offender legislation, probation orders and life licence, Ministry of Justice procedures and child protection legislation
Detailed operational knowledge of secure services, including risk assessment/management
Evidence of post qualifying and continuing professional development
Must understand the background to and aims of current healthcare policy and appreciate the implications of this on engagement
Must be able to prioritise own work effectively and be able to direct activities of others
Experience of managing and motivating a team and reviewing performance of the individuals
Development and maintenance of therapeutic alliance with service users and their families/carers
High-level management skills, able to perform to high level in clinical setting, holding services to account and ensuring effective use of PC resources
Must be able to provide and receive highly complex, sensitive or contentious information, negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups
Demonstrable commitment to and focus on quality, promotes high standards to consistently improve patient outcomes
Applicants must have current UK professional registration
Able to travel across the PC region as well as national travel requirements on a regular basis
Nice to have:
Educated to masters level or equivalent level of experience of working at a senior level in specialist area
