Job Responsibility

Assessing the needs of adults and their Carers to identify creative ways to meet their assessed outcomes under the Care Act 2014
Working across admission avoidance, acute hospital, inpatient settings, and rehabilitation and reablement services
Supporting discharge planning with a home first approach
Visiting people in their own home to support them back to independence using a strengths-based approach
Conducting outcome led interventions using a strength-based approach
Assessing clients’ needs and agreeing a person-centred support plan
Completing reviews of assessments and support plans
Supporting people with their health, mental health, and social care needs
Assessing mental capacity when required
Working independently and managing own caseload
