The Great Western Hospitals NHS Foundation Trust is seeking a motivated Medicines Optimisation Pharmacy Technician to join our dynamic and supportive clinical pharmacy team. In this role, you will play a key part in ensuring safe, effective, and efficient use of medicines across our hospital. Working closely with pharmacists, nursing staff, prescribers and wider clinical teams, you will support high-quality patient care by delivering accurate medicines reconciliation, improving medicines safety, and helping patients get the most from their medicines. The pharmacy team are forward thinking and continuously strive to improve our ways of working, putting medicines optimisation at the heart of our clinical services. We put the patient at the center of our care and aim to enhance every patient's experience.
Job Responsibility:
Carrying out timely and accurate medicines reconciliation for patients on admission
Supporting safe and effective discharge through processing discharges at ward level and counselling patients about their medicines
Participating in ward-based medicines optimisation initiatives to reduce medication errors and improve outcomes
Helping implement best practice, formularies, and medicines policies across the Trust
Supporting medicines supply, stock management, and minimising waste
Using digital systems to improve the flow of medicines information and support clinical decision making
Acting as a role model for high-quality, patient-centred medicines management
Requirements:
Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC)
Experience in hospital or primary care pharmacy
Confident communicating with patients and multidisciplinary teams
Organised, proactive, and able to work independently as well as part of a team
Passionate about improving patient safety and medicines optimisation
Working within a team
Managing own workload to meet the needs of a service
Working with Pharmacy computer systems
Previous experience of training others
Knowledge of pharmaceutical technical procedures acquired through training, NVQ3, BTEC in pharmaceutical science or equivalent
Additional training for checking dispensed prescriptions, supervising a technical area within the pharmacy to degree or equivalent level
Knowledge of medicines management, including relevant legislation and policies acquired through NVQ3 or BTEC equivalent level qualification plus specialist training and experience to degree or equivalent level
Nice to have:
Working in a busy Dispensary
In-depth post qualification experience in a hospital pharmacy
