Job Description:

At CVS Health, you will proactively support high-need patients, navigate insurance benefits, and empower patients to manage their conditions. You will collaborate with care teams and Community Health Workers, providing care coordination and education on supportive care while maintaining extensive knowledge of community resources.

Job Responsibility:

Proactively support high-need patients by providing assessments, telephonic check-ins, and in-person visits
partner with patients to identify goals and increase healthcare engagement
maintain knowledge of community resources and make referrals as needed
navigate insurance benefits
provide care coordination and empower patients to manage health conditions
educate patients on supportive care options and provide advocacy
provide psychoeducation to patients and families
meet with care teams to generate solutions
respond to crises with prioritization and multitasking
collaborate with Community Health Workers

Requirements:

Extensive knowledge of community resources
ability to provide comprehensive assessments, telephonic check-ins, and in-person visits
ability to navigate patient insurance benefits
ability to provide psychoeducation to patients and families surrounding life stage and disease-specific issues
ability to respond to crises and urgent needs
ability to provide clinical guidance and consultation to Community Health Workers
ability to complete concise and comprehensive documentation on time
