Job Description

Medical Social Worker position in Oncology at Advocate Christ Medical Center. Full-time, on-site role providing discharge planning and counseling services to patients and families in a hospital setting.

Job Responsibility

  • Discharge planning for hospital inpatients
  • Provision of counseling services to patients and families
  • Timely and complete documentation to meet licensure and third party payor requirements
  • Provision of in-services to assigned units and attendance of educational seminars as appropriate
  • Preparation of statistical records
  • Functions as a member of the care management thematical situation
  • Contributes to Continuous Process Improvement activities

Requirements

  • MSW from accredited school of Social Work and field experience within a hospital setting
  • Must recognize age specific needs and behaviors
  • Must adhere to NASW code of ethics and standards
  • IL- LSW required, LCSW preferred
  • Demonstrates competence in counseling and discharge planning
