Job Description:

The role of a Medical Scribe at Oak Street Health involves supporting primary care providers with clinical documentation, assisting in the patient care journey, and ensuring accurate and specific medical records. The position provides extensive training in clinical workflows, value-based medicine, and chronic condition care.

Job Responsibility:

Support primary care providers with clinical documentation
Assist providers with patient care journey including planning, observing, documenting, and following up
Ensure accurate, specific, and timely clinical documentation
Identify and help close care gaps
Receive training in clinical workflows and preventative care
Become an expert in value-based care models and chronic condition care
Partner closely with assigned provider during patient interactions.

Requirements:

Commitment of at least 1-2 years to the role
Expertise in value-based care models and documentation
Knowledge of chronic condition care, including ICD-10 and CPT coding
Strong communication skills
Ability to follow clinical workflows and team-based care
Interest in careers such as Health Informatics, Public Health, Healthcare Administration, Medical Coding, or related fields
Willingness to receive on-the-job training.

Nice to have:

Experience in clinical settings
Interest in MD/DO/PA/NP programs
Knowledge of population health data streams
Familiarity with preventative care for chronic conditions.
