We are looking for a Medical Records Technician to support the integrity and organization of resident health information in Portola Valley, California. In this role, you will help maintain complete, timely, and regulation-ready records while partnering with clinical and administrative teams to secure missing documentation and resolve inconsistencies.
Job Responsibility
Review resident files related to admissions, transfers, and discharges to confirm completeness, accuracy, and adherence to healthcare regulations
Examine clinical materials such as physician documentation, medication administration records, laboratory results, charts, and treatment notes for consistency and proper filing
Work closely with nurses, physicians, and outside care providers to gather outstanding records and ensure required documentation is received promptly
Maintain orderly paper and electronic record systems and update information accurately within the organization's EHR platform
Investigate documentation issues, correct record discrepancies, and support reporting activities tied to compliance and health information management
Assist with coding-related record review and help prepare documentation for audits, inspections, and internal quality checks
Requirements
At least 2 years of experience in medical records, health information management, or a related healthcare support role
Hands-on experience working with electronic health record systems in a clinical or long-term care environment
Familiarity with platforms such as AHLTA, Allscripts, Cerner, or similar healthcare information systems
Strong understanding of medical documentation standards, record accuracy practices, and regulatory compliance requirements
Ability to review charts, graphs, and clinical records with a high degree of precision and attention to detail
Effective communication skills with the ability to coordinate professionally with clinical staff and external providers
Strong organizational skills and the ability to manage multiple record requests and deadlines in a fast-paced setting
