Job Description:

We are looking for a detail-oriented Medical Records Clerk to support a project working with patient charts, medical coding, and reviewing past appointments. This role involves conducting a thorough review of patient records, ensuring coding accuracy, and verifying documentation for a local healthcare organization. If you have experience in medical records, chart audits, and coding, we encourage you to apply.

Job Responsibility:

Conduct detailed reviews of patient records, focusing on chart notes and coding accuracy
Audit the last month's appointments to verify proper documentation and billing levels
Ensure coding aligns with new versus established patients and the office visits billed
Input findings and coding details into spreadsheet for reporting and analysis
Use ICD-10 and CPT codes to ensure compliance with medical standards
Cross-check patient visit details, such as prior appointments or visits to other locations, to maintain data accuracy
Collaborate with supervisors and team members to address discrepancies or concerns identified during audits
Utilize electronic medical record (EMR) systems to access and review patient charts
Maintain a high level of confidentiality and accuracy while managing sensitive patient information

Requirements:

Proven experience in medical records review, chart audits, or related fields
Familiarity with medical coding systems, including ICD-10 and CPT codes
Proficiency in using electronic medical record (EMR) systems
Strong attention to detail and ability to identify errors or inconsistencies in documentation
Knowledge of billing practices for new and established patients in a medical setting
Ability to analyze data and compile findings into clear, organized reports
Excellent organizational and communication skills
Understanding of confidentiality requirements and HIPAA regulations
