The Medical Director, Population Health will report to the Senior Medical Director, Population Health - Complex Patient Care and will play a pivotal role in shaping and guiding our clinical programs for patients who are at greatest risk for poor outcomes due to chronic conditions, behavioral health challenges, and social barriers to care. This clinician leader will design, implement, and continually improve the systems and strategies that help Oak Street clinicians care for our most complex patients—those whose health is influenced by overlapping medical, psychological, and social factors. The role spans clinical innovation, data-informed program design, and collaborative leadership to ensure every complex patient receives coordinated, compassionate, and effective care. The Medical Director will work cross-functionally with operations, analytics, technology, and regional care teams to create an integrated approach that improves outcomes, prevents avoidable hospitalizations, and helps patients achieve their fullest health potential.

Requirements:
Authorization to work in the U.S.
M.D., D.O., APN, or PA with active clinical license and board certification in Internal Medicine, Family Medicine, Preventive Medicine, or Geriatrics
Demonstrated experience leading programs or teams focused on complex or high-risk patient populations
Strong familiarity with managed care, value-based reimbursement, and population health management
Proven ability to analyze and apply data to guide program improvement and measure clinical outcomes
Exceptional collaboration and communication skills, with the ability to influence across disciplines and functions
Nice to have:
Advanced training or degrees (e.g., Fellowship, MPH, MBA) and/or experience in management consulting preferred
