The Medical Director will be responsible for leading, as a dyad partner with the Mobile Practice Manager, the in-home primary care provider teams in the specified geography. The Oak Street Health At Home division is an interdisciplinary team that focuses on primary care and transitional care support for our highest-need patients in their homes. The primary responsibility for the Medical Director is direct management, oversight, and coaching of the local team, and is part of the At Home division's leadership team which helps develop programs (i.e., complex care coordination, chronic disease management, psychosocial support, etc.) throughout the division. This entails collaboration with the clinical teams in the center, medical directors, and other partners outside of Oak Street Health. The Medical Director will see patients in their homes up to 3 days a week alongside their provider colleagues. Additionally, we see this leader being a key thought partner in providing feedback on our clinical model especially as it pertains to the At Home Program.

  • Drive Culture: At Oak Street, we foster a culture of innovation and open dialogue, and we work with urgency. This requires passion and enthusiasm, active listening skills, and comfort with and commitment to regular change. Our Medical Directors are expected to support this culture and set an example for others to follow
