Job Description:

The Medical Assistant/CIS, House Calls Program role at Oak Street Health is focused on supporting logistics, clinical care, and documentation in home-based care programs. The position involves accompanying nurse practitioners on house calls, taking vital signs, medication reconciliation, patient/family education, and documentation, among other duties. Oak Street Health is dedicated to high-quality primary care for Medicare patients and operates on a community-centered healthcare model.

Job Responsibility:

  • Preparing visit schedule, route, and logistics for all visits
  • Accompanying nurse practitioner on all house calls
  • Vital signs, medication reconciliation, patient / family education
  • Documenting House Call note, managing referrals, orders and follow-up
  • Maintaining supplies/equipment
  • Other duties as assigned

Requirements:

  • At least one year experience as a Medical Assistant + MA certification
  • Must pass all clinical competencies before supporting patient care, phlebotomy a plus
  • Daily access to a vehicle with valid license and insurance
  • Flexibility to travel around Chicagoland
  • Bilingual (English and Spanish) a plus
  • US work authorization
