Job Description:

LeRoy Village Green Residential Healthcare Facility, Western New York's trusted provider of short-term rehabilitation and long-term care, is seeking a highly skilled and compassionate MDS Assistant- Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our dedicated team. As an MDS Assistant, you will play a critical role in ensuring the delivery of high-quality care to our residents by assisting with the Minimum Data Set (MDS) process, conducting assessments, and developing individualized care plans.

Job Responsibility:

  • Assist with the completion of MDS assessments, identifying and prioritizing resident care needs
  • Conduct thorough and accurate resident assessments, using observation, interview, and physical examination skills
  • Develop and implement care plans in collaboration with the interdisciplinary team, ensuring resident-centered care
  • Collaborate with nursing staff to implement care plans, monitor resident progress, and make necessary adjustments
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives to drive continuous quality improvement
  • Maintain accurate and detailed records of resident care and assessments
  • Provide exceptional customer service, respecting the unique needs and preferences of each resident

Requirements:

  • Current registration as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of New York
  • Prior experience, with a focus on long-term care or rehabilitation settings
  • Strong assessment and care planning skills, with the ability to work effectively in a fast-paced environment
  • Excellent communication and interpersonal skills, with the ability to build rapport with residents, families, and colleagues
  • Ability to work flexible hours, including evenings, weekends, and holidays
