The Manager, Population Health is a key contributor to the success of Oak Street Health’s clinical model, with a focus on improving health outcomes and managing medical costs. This role leads strategic and operational initiatives, including care program development, clinic support, and stakeholder engagement. Responsibilities span program implementation, reporting and analytics, and evidence-based care design. The position reports to the Director of Population Health Strategy & Analytics and offers significant growth potential.

Job Responsibility:

  • Drive initiatives focused on improving health outcomes and controlling medical costs in our patient population
  • Own the strategy, process, and implementation of key operational roll-outs, including on-the-ground support at clinics
  • Lead the clinical development of new care programs by coordinating feedback from care teams and reviewing evidence-based guidelines
  • Provide reporting and analytics to support Care Teams (physicians, nurses, care managers)
  • Establish and maintain relationships with internal and external stakeholders
  • Perform other duties as assigned

Requirements:

  • 7–10 years of relevant experience, including a minimum of 2 years of direct patient care
  • Strong problem-solving, communication, and analytical skills
  • Bachelor’s degree preferred
