Manager, Patient Engagement--Patient Engagement Escalation Program

At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality and cost-effective care. The Manager, Population Health reports to the Director of Patient Engagement. The ideal candidate is a self-starter, enjoys solving challenging problems, uses data to drive insights, and excels in a fast-paced environment. This will be a high-visibility role requiring significant cross-team collaboration within Population Health and other corporate teams (e.g., Business Intelligence, Care Services, Clinical Quality, Managed Care Operations, etc.).
