Job Description:

The Manager of Population Health will immediately become an important contributor to the Oak Street Health team. S/he will execute a set of responsibilities to enable the success of our clinical model, which is core to Oak Street's success.

Job Responsibility:

Key focus on health outcomes and controlling medical costs in our patient population
Owning the strategy, process, and implementation of key operational roll-outs (will involve on-the-ground support at our clinics)
Leading the clinical development of new care programs, including coordinating and synthesizing feedback from care teams and reviewing evidence-based guidelines
Reporting and analytics in support of our Care Teams (physicians, nurses, care managers)
Establishing and maintaining relationships with both internal and external stakeholders
Other duties, as assigned

Requirements:

Track record of running through walls to solve difficult problems
Orientation toward analytical problem solving
Practical, results-oriented mindset
Ability to manage multiple priorities while maintaining a positive attitude and sense of humor
Unimpeachable integrity and maturity
2+ years of experience with direct patient care
Strong MS Excel skills
US work authorization
