Job Description
This is an exciting opportunity to join the nurse led corporate Macmillan Specialist Palliative Care team to provide expert care to adult in-patients across the Trust who have complex symptoms and specialist palliative care needs, advanced care planning requirements , or complex end of life care. The SPC team work a 7 day service (approx.1 in 5 weekends) to provide standalone CNS advice at the weekends with telephone support available from off site rotaed Palliative Medicine Consultant/ Registrar staff. The team provide high level support and manage a variable case load of patients working together Mon Fri /and then cover a standalone weekend SPC service, whilst supporting the weekend triage of Acute Oncology patients. Patient care is always patient centred and we expect applicants to have a solid background of palliative care/ cancer experience and education to MSc level. The SPC service is managed by the Trust Lead Cancer Nurse.
Job Responsibility
The SPC CNS team manage new and existing referrals of adult patients with complex palliative care needs
The SPC CNS manages individual cases assessing individual needs, advising on symptom management needs and sigposting to other service
Attending the weekly SPC MDT
Managing the education needs of generalist staff
Attending the Trust SPC/ EOLC Steering Group and contributing to discussions
Requirements
RGN extensive post-registration experience of which a significant amount worked at Band 6 or equivalent within relevant clinical environment
Diploma in Palliative and supportive care
Possession of a relevant degree
Assessing and mentoring or teaching qualification ie ENB 998 or equivalent
Counselling/ advanced comms qualification or training to add
Understanding of the need for confidentiality
Expert knowledge relevant to the speciality/department/field of practice
Understanding of professional and current issues in nursing and healthcare, e.g. NSFs, NHS Plan, NHS Cancer Plan and service improvement agenda
Comprehensive understanding of audit and research methodologies
Understanding of resource management, health and safety, clinical risk and quality issues
Awareness of equality and diversity
Clinical Governance
Is able to articulate a personal philosophy of nursing and understanding of clinical nurse specialism
Demonstrates understanding of the role and fit of the CNS within service provision
Senior Nurse experience in palliative care
Experience within either hospital or community setting
Experience of leading and managing a team of nurses
Previous experience of research and quality audit including its application
Previous experience of change management and problem solving
Experience of handling clinical incidents and complaints
Nice to have
Non Medical Prescriber qualification or willingness to work towards
Relevant Masters degree or in the process of completing
