Job Description:

We are seeking a compassionate and detail-oriented LVN/LPN Case Manager to support patient care coordination across various healthcare settings. The LVN/LPN will assist with overseeing patient care plans, monitoring progress, educating patients and families, and collaborating with the interdisciplinary team to ensure timely, effective, and patient-centered care.

Job Responsibility:

Support the development and implementation of individualized care plans based on patient needs
Coordinate patient services, follow up on care progress, and communicate updates to the care team
Monitor patient conditions, document relevant clinical information, and report significant changes promptly
Educate patients and families on treatment plans, medications, and home care instructions
Facilitate communication between patients, providers, and healthcare teams to ensure continuity of care
Assist with discharge planning, transitions of care, and resource coordination
Maintain accurate and timely documentation in accordance with clinical guidelines

Requirements:

Current LVN/LPN license (active and unrestricted)
Experience in case management, home health, long-term care, or outpatient care preferred
Strong understanding of nursing principles, care planning, and patient education
Excellent communication, organization, and follow-through skills
Compassionate, dependable, and able to work collaboratively within a multidisciplinary team
