The Home Health Licensed Practical Nurse (LPN) is a licensed professional who administers personal care and routine skilled nursing care in the home to clients under the direction of a Registered Nurse. Collaborate with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to implement and follow a patient specific care and treatment plan. The LPN reports to the Nurse Manager.
Job Responsibility:
Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique
Implements plan of care initiated by the registered nurse
Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care
Provide accurate and timely documentation consistent with the plan of care
Participation in Case Conferences
Provides direct patient care as defined in the State Nurse Practice Act.
Perform vital signs and head to toe patient assessment during visits
Manage and perform wound care treatment (including but not limited to surgical wounds, Wound Vac)
Provide assessment, execute and manage Central Line/Peripheral line, urinary catheters, colostomy, PleurX Drains, Lab Draws
Requirements:
LPN- Licensed Practical Nurse
BLS-Basic Life Saving Certification
LPN program graduate
