Job Description:

The Home Health Licensed Practical Nurse (LPN) is a licensed professional who administers personal care and routine skilled nursing care in the home to clients under the direction of a Registered Nurse. Collaborate with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to implement and follow a patient specific care and treatment plan. The LPN reports to the Nurse Manager.

Job Responsibility:

  • Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique
  • Implements plan of care initiated by the registered nurse
  • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care
  • Provide accurate and timely documentation consistent with the plan of care
  • Participation in Case Conferences
  • Provides direct patient care as defined in the State Nurse Practice Act.
  • Perform vital signs and head to toe patient assessment during visits
  • Manage and perform wound care treatment (including but not limited to surgical wounds, Wound Vac)
  • Provide assessment, execute and manage Central Line/Peripheral line, urinary catheters, colostomy, PleurX Drains, Lab Draws

  • LPN- Licensed Practical Nurse
  • BLS-Basic Life Saving Certification
  • LPN program graduate
