Are you a passionate and committed Qualified Social Worker with experience supporting care-experienced young people? We are currently recruiting for an experienced Leaving Care Social Worker to join a supportive and forward-thinking Leaving Care Team in Halton. This is a rewarding opportunity to work with young people as they transition into independence, helping them achieve positive and sustainable outcomes.
Job Responsibility:
Manage a caseload of care-experienced young people
Develop and review Pathway Plans in line with statutory requirements
Support young people in areas such as accommodation, education, training, and employment
Promote independence and life skills development
Work collaboratively with partner agencies and professionals
Maintain accurate and timely case recordings and reports
Advocate for young people, ensuring their voice remains central
Requirements:
Diploma / Degree in Social Work
Social Work England registration
Experience working with Leaving Care / Care-Experienced Young People
Strong understanding of relevant legislation and statutory duties
Ability to build effective relationships with young people
Good organisational and communication skills
Ability to work independently and as part of a team
