Job Description:

Hunter Gatherer AHP have a fantastic locum opportunity for an experienced Primary Care Network Pharmacist to join a well-established service based in Surrey. If you’re passionate about improving patient outcomes in a primary care setting and have a strong background in chronic disease management – particularly diabetes – this could be the perfect role for you.

Job Responsibility:

Conducting in-depth clinical reviews and managing long-term conditions
Developing tailored care plans for patients with complex needs
Supporting diabetes management initiatives across the PCN
Working collaboratively with GPs, nurses, and other healthcare professionals
Ensuring accurate documentation and continuity of care

Requirements:

GPhC-registered Pharmacist
Independent Prescriber (IP) qualification
Strong experience in primary care or GP practice settings
Proven ability to manage patients with diabetes and other LTCs
Confident working autonomously and within a wider team
Committed to clinical excellence and continuous improvement
