Job Description:

Conducting medication reviews and optimising treatment plans

Requirements:

GPhC registration (essential)
Experience in hospital-based clinical pharmacy (Band 7 or 8a level)
Strong knowledge of NHS prescribing guidelines & protocols
Excellent communication & problem-solving skills
Ability to work independently & within a team
