• Excellent communication, training, referral, and planning skills
• Two-year recent experience with elders, those diagnosed with ID/D, ABI, or chronic conditions affecting ability to live safely in the community
• Strong interpersonal and communication skills
• Ability to work collaboratively in a team environment
• Background Check Required
• Valid driver's license with an excellent RMV record

Community experience that expands specifically to the Department of Developmental Service, Adult Foster Care Services, HCBS waivers (supporting those with a Brain Injury or those who have chronic conditions affecting their ability to live safely in the community)
