Job Description:

Job Responsibility:

Assisting in the development, review, and revision of a care plan
Providing nursing care in accordance with the physician’s plan of treatment, and individual plan of care, as authorized by the client’s payer source
Observing, recording, and reporting reaction to treatment and any changes in client’s condition to appropriate personnel and/or physician
Providing ongoing instruction and supervision of client, family, or other health team personnel in the plan of treatment
Participating in education programs, quality assessment activities, nursing meetings, and other staff meetings as required

Requirements:

One year of experience in a home health care setting preferred
Active LPN license in good standing
Driver’s license and vehicle with current auto insurance
Positive, professional attitude
