We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Job Responsibility:
Provide direct and indirect nursing care to adult and geriatric residents under the supervision of an RN
Collect and assess data related to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team
Contribute to and support individualized, age-appropriate, and culturally sensitive plans of care
Monitor resident outcomes and recommend revisions to care plans as needed
Collaborate with residents, families, physicians, and interdisciplinary team members to promote optimal outcomes and satisfaction
Maintain current clinical knowledge through continuing education and in-service training
Assist in mentoring and orienting new staff
Demonstrate leadership, professionalism, and adherence to safety and infection control standards
Perform duties with awareness of potential exposure to bloodborne pathogens
Requirements:
Active New York LPN License
BLS Certification (AHA or RQI only)
Minimum 1 year of experience as an LPN
LTC experience preferred
Ability to work rotating shifts and holidays
Nice to have:
LTC experience preferred
Passion for working with the geriatric population
