Licensed Practical Nurse

• Providing direct nursing care as prescribed by the physician to acutely ill, and/or chronically ill patients in their homes
• Recording clinical and progress notes
• Coordinating other patient care activities/services to provide the highest quality of patient care
• Provides direct skilled nursing care under the supervision of a Registered Nurse and in accordance with the plan of care
• Utilizes the E.V.V. (Electronic Visit Verification) program as applicable
• Prepares equipment and material for use according to the plan of care, practicing appropriate procedure technique, including Administers medications as prescribed by a physician and permitted by law
• Assists with teaching the patient and/or family members regarding the patient care needs and other related problems of the patient at home
• Ensures confidentiality of patient records/information
• Attends team meetings and staff meetings to enhance team communication and coordination of quality services
• Observes, records, and reports (1) signs and symptoms
• (2) reactions to treatments and medications
• and (3) any change in the patient's physical or emotional condition in a timely matter to the Registered Nurse and physician as applicable
• Participates in the organization's Performance Improvement and Compliance programs and adheres to policies and procedures.

• Must meet educational/experience requirements outlined in the laws of licensure states
• A minimum of one (1) year of acute care experience preferred
• previous home care experience preferred
• Must have current LPN licensure in state of practice
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation
• Current CPR certification required
• LA: Current LPN Licensure in LA without restrictions. At a minimum, one (1) year of experience working as an LPN.
