This LPN is needed to provide hands on nursing coverage for the individuals with varying medical needs who reside in Meadow Lane IRA and Rudd Pond IRA. Meadow Lane IRA will serve as the primary location for this position and accommodates five individuals with varied medical and behavioral requirements. The Rudd Pond IRA provides care for four individuals with different nursing needs, including tasks that cannot be delegated to unlicensed direct care staff. Key responsibilities will include developing patient care protocols and training staff on necessary care procedures. The LPN will ensure compliance with patient care standards and provide services in a timely and appropriate manner, according to individualized treatment plans. Additionally, the LPN will evaluate patients’ responses to treatments and medications, as well as any changes in their health status, and will promptly communicate these observations to a Nurse II or physician. The LPN will address concerns raised by patients, families, and staff, escalating issues to higher-level personnel when necessary. Maintaining accurate documentation of patients’ conditions and the treatments administered is also a critical aspect of this role. The LPN may be asked to provide coverage at other sites within the team if care is needed. Nursing oversight will be provided by the Registered Nurse 1, with administrative oversight provided by the Registered Nurse Supervisor 1.
Job Responsibility:

Assist with nursing tasks within their scope of practice
Assist with individuals’ personal hygiene and daily living tasks
Ensure medications and treatments are performed and data recorded
Maintain tickler for OTC expiration dates and orders
Monitor/supervise individuals with insulin administration and PO daily medications/treatments
Provide first aid and monitor vital signs
Act as liaison between IRA Residential staff and the Registered Nurse 1
Purchase and organize med room supplies
Requirements:
A license and current registration to practice as a practical nurse in New York State
OR A limited permit to practice as a practical nurse in New York State
OR An application on file for a limited permit to practice as a practical nurse in New York State
A valid Driver’s license
