Job Description:

We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) to join our home care team. As an LPN, you will provide one-on-one nursing care to clients in their homes, assisting with medical needs, monitoring health conditions, and ensuring the highest standard of care.

Job Responsibility:

Provide direct patient care in a home setting
Administer medications and treatments as prescribed
Monitor and document patient health status and report changes to RNs or physicians
Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming
Educate clients and their families on health conditions and proper care techniques
Ensure compliance with care plans and follow all healthcare protocols
Collaborate with caregivers, families, and healthcare professionals to optimize patient outcomes

Requirements:

Active Florida LPN License
CPR Certification (American Heart Association or Red Cross preferred)
Minimum of 1 year of nursing experience (home care experience preferred)
Strong clinical and assessment skills
Compassionate, patient-focused, and reliable
Ability to travel to client homes within assigned locations (Port St. Lucie, Cutler Bay, Fort Lauderdale)
