Licensed Practical Nurse

• Assist the RN in patient care by completing assigned patient care tasks
• Collect patient healthcare data systematically
• Administer medications and vaccinations (including injections)
• Provide telephonic support for patients
• Monitor and coordinate delivery of the care plan
• Form relationships with patients and their caregivers
• Conduct medication and testing authorizations
• Deliver an exceptional patient experience

• Current state license to practice as an LPN
• CPR Certification required
• EMR experience
• Strong communication skills and customer service orientation
• A problem-solving orientation, accountability, and a flexible and positive attitude
• A supportive attitude toward our patient population
• 2 or more years of healthcare experience working in a primary care office or other outpatient setting
• US work authorization
