Job Description

As a Licensed Practical Nurse (LPN) at Premier Washington Health Center, you will play a vital role in providing high-quality patient care in our rehabilitation and nursing facility.

Job Responsibility

  • Provide direct patient care under the supervision of a registered nurse or other licensed healthcare professionals
  • Administer medications and treatments as prescribed by physicians
  • Monitor patients' conditions and report any changes or concerns to designated staff
  • Assist with admissions, transfers, and discharges as needed
  • Maintain accurate and up-to-date patient records and charts
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Provide skilled nursing care to patients with varying needs and conditions
  • Perform routine vital signs and other assessments as needed
  • Administer injections, medications, and treatments as prescribed
  • Assist with wound care, dressing changes, and other medical procedures
  • Document patient progress and maintain accurate records
  • Participate in quality improvement initiatives to improve patient outcomes

Requirements

  • Current LPN licensure in the state of Pennsylvania
  • Prior experience preferred, we welcome new graduates as well
  • Strong communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Commitment to providing exceptional patient care and customer service
  • Minimum 1 year of experience (based on 'Minimum Years of Experience' field)
