Job Description:

Rosewood Rehabilitation & Nursing Center is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Weekend Baylor Program team! As an LPN, you will play a vital role in providing high-quality patient care to our residents, under the supervision of our experienced healthcare team. Our Weekend Baylor Program is a unique and rewarding opportunity to work with our residents on weekends, with flexible scheduling options.

Job Responsibility:

  • Provide direct patient care, including administering medications
  • Monitor and report changes in patient condition to the registered nurse or physician
  • Collaborate with the interdisciplinary team to develop and implement individualized patient care plans
  • Maintain accurate and detailed records of patient care and treatments
  • Participate in quality improvement initiatives to optimize patient outcomes and satisfaction
  • Foster a positive and respectful environment that promotes patient-centered care

Requirements:

  • Current LPN licensure in the state of Pennsylvania
  • High school diploma or equivalent required
  • some college or nursing program education preferred
  • Prior experience in a healthcare setting
  • previous experience in long-term care or rehabilitation setting a plus
  • Strong communication and interpersonal skills
