The Licensed Practical Nurse Care Manager (LPN-CM) is the primary onsite licensed leader responsible for coordinating and supporting the delivery of resident care in accordance with the ANBLPN Standards of Practice, Code of Ethics, and New Brunswick Social Development Guidelines. The LPN-CM provides clinical oversight within the LPN scope, supervises unregulated care providers, ensures safe and evidence-informed practice, maintains accurate documentation, and promotes respectful, resident-centered care. The role requires strong leadership, sound clinical judgment, and effective communication with residents, families, interdisciplinary partners, and the management team.
Job Responsibility:
Develop, review, and update individualized care plans in collaboration with residents, families, and the interdisciplinary team
Conduct assessments within LPN scope, identify changes in condition, and communicate promptly with the physician, Nurse Practitioner (NP), or Extra-Mural as required
Complete accurate documentation during admissions, transfers, discharges, and status changes
Monitor and document activities of daily living (ADL), behaviors, risks, and overall health status, ensuring care plans reflect current needs
Implement interdisciplinary recommendations and ensure care tasks are completed by staff
Conduct daily rounds to monitor resident well-being, support staff, review care delivery, and identify follow-up actions
Participate in on-call responsibilities and respond to after-hours concerns within scope
Maintain and update professional knowledge through ongoing education
Deliver or coordinate staff education, including orientation, safe handling, dementia care, infection control, and updated protocols
Liaise with internal and external educators to ensure staff meet mandatory training requirements
Facilitate regular staff meetings to communicate expectations, review clinical issues, share updates, and reinforce best practices
Support a positive, collaborative environment through team building and coaching
Provide daily onsite leadership as the sole licensed professional, ensuring safe practice, appropriate delegation, and clear communication
Supervise unregulated care providers, ensuring tasks are completed in accordance with care plans and standards
Address staff concerns, performance issues, or workflow challenges in collaboration with the Director of Operations
Accountable for oversight of staff scheduling to ensure that staffed hours meet resident care needs and licensing requirements
Promote resident dignity, privacy, autonomy, and confidentiality in all interactions
Assist with transporting and transferring residents to appointments, as needed
Monitor care delivery to ensure it meets resident needs, organizational policies, and regulatory requirements
Ensure safe medication administration practices within LPN scope and escalate concerns appropriately
Review incidents, changes in condition, and safety events to support timely follow-up and corrective actions
Identify equipment issues, environmental hazards, or safety risks and report them promptly for corrective action
Ensure documentation—including progress notes, incident reports, and audits—is accurate, complete, and timely
Maintain open, respectful communication with residents and families
respond to concerns promptly and professionally
Advocate for resident needs and support family understanding of care plans and changes in status
Facilitate communication between families and the management team as needed
Support the Director of Operations through effective communication, coordination, and teamwork across departments
Ensure staff adherence to health, safety, and infection control procedures
Requirements:
Successful completion of a recognized Practical Nursing Program
Active registration with the Association of New Brunswick Licensed Practical Nurses (ANBLPN)
3–5 years of LPN experience in care coordination or long-term care
Leadership or supervisory experience preferred
Experience with Alzheimer’s disease and other dementias
Back in Form or equivalent safe-handling certification
Current First Aid and CPR certification
Strong leadership, communication, and critical-thinking skills
Ability to function independently as the sole onsite licensed professional
Ability to work effectively in a fast-paced environment
Proficiency in Microsoft Office and electronic documentation systems
Valid driver’s license
