Job Responsibility

  • Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans
  • Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence
  • Monitor and document residents’ clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes
  • Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care
  • Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff

Requirements

  • Active, unencumbered MA Licensed Practical Nurse (LPN) licensure
  • Strong clinical skills in basic nursing procedures, including medication administration, vital signs monitoring, and documentation in electronic health records (EHR)
  • Knowledge of long-term care regulations, infection control standards, and patient safety protocols in a skilled nursing or similar healthcare setting
  • Current CPR certifications, including BLS and ACLS
  • Ability to communicate fluently in English – both verbally and written
