We are seeking a licensed or provisionally licensed behavioral health professional in New York State to provide clinical oversight, direct support, and structured coordination of care within a community-based setting. Job Responsibility:
Collaborate with the Clinical and Wellness Coordinator to ensure PHQ-9 screenings are completed and reassessed as needed
Conduct in-person follow-up for members scoring 10 or higher on PHQ-9 assessments
Develop and implement safety plans for members identified as high risk or reporting self-harm history
Create individualized service plans for members engaged in clinical mental health care
Provide clinical oversight and guidance to the Clinical and Wellness Coordinator
Maintain open communication with supervisors, direct care staff, family members, guardians, and collateral contacts regarding member status and progress
Participate in Interdisciplinary Team Meetings and other relevant care coordination meetings
Submit monthly and quarterly progress notes in accordance with agency and funder requirements
Attend monthly, quarterly, and MHI meetings for professional development, coaching, and care review
Provide annual and as-needed in-service training to ensure staff competency in program plan implementation and documentation standards
Maintain comprehensive and audit-ready record keeping including service plans, training documentation, notes, and contact records
Assist in developing and refining training procedures and program materials to ensure operational alignment with established regulatory guidelines
Stay current with behaviorally based interventions and therapeutic techniques to maintain best practice standards
Provide crisis intervention documentation and weekly reporting to supervisor
Perform additional related duties to ensure efficient program operations
Requirements:
Must be licensed or provisionally licensed in the State of New York as one of the following: LMSW, Licensed Master Social Worker
LCSW, Licensed Clinical Social Worker
LMHC, Licensed Mental Health Counselor
Master's degree in Social Work, Counseling, or related Behavioral Health field
1 to 2 years of experience in a behavioral health setting preferred
Strong knowledge of regulatory and licensing standards
Proficient in Microsoft Word, Excel, and electronic documentation systems
Demonstrated ability to manage confidential information professionally
Excellent communication, organizational, and interpersonal skills
Ability to work effectively in a fast-paced, sometimes high-stress environment
Flexible and adaptable to evolving priorities and clinical demands
