Job Responsibility

Provide individual and group therapy to oncology patients and caregivers
Conduct comprehensive psychosocial assessments
Deliver crisis intervention and emotional support
Facilitate advance care planning discussions
Address non-medical barriers to care, including transportation, financial stressors, and access to community resources
Collaborate as a key member of the interdisciplinary team to develop and implement patient-centered care plans
Participate in quality improvement initiatives and program development
Provide comprehensive psychosocial care to oncology patients and their families in an outpatient setting
Deliver longitudinal support across the cancer care continuum, including diagnosis, active treatment, survivorship, and end-of-life care
Play an integral role in supporting patients receiving palliative and supportive care services

Requirements

Master's degree in Social Work
Licensed to practice within the Commonwealth of Pennsylvania
At least five (2) years of experience providing mental health treatment
Strong knowledge of community resources and social services
A background check in accordance with state and federal regulations
State Criminal Clearance, Child Abuse Clearance and FBI Clearance
NPI Query and current driver's license
Participation in and completion of all CBH mandatory in-services
Excellent writing and verbal communication skills
Computer proficiency and the ability to provide telehealth
