Job Responsibility

Working in a multidisciplinary collaborative team with physicians and other healthcare team members in determining patient’s medical status, treatment plan and prognosis as they relate to and effect psychosocial care, long-term care, palliative and end-of-life care planning
Actively participating in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all
Assessing psychosocial needs and provides services that enhance patient functioning, emotional well-being and continued access to services and resources in the community
Providing care coordination

Requirements

Clinical licensure in their field in OH (e.g. LPC, LPCC, LSW, LISW, MFT, IMFT)
Ability to demonstrate and model stable, appropriate boundaries with clients
Ability to complete and submit documentation of services and other documents within 24 hours
Comfort and familiarity working with a diverse client base
Proficient in the use of Office 365 and Electronic Health Record systems (Valant experience a plus!), including the collection of co-payments

Job Responsibility

Diagnose, create and maintain individualized treatment plans
Provide excellent customer service for clients and collaborate with a dynamic team to further the mission of filling gaps in our community
Utilize creativity in interventions to help clients achieve and exceed goals
Maintain clinical documentation in EHR system
Maintain a caseload of a minimum of 25 client visits per week (for full time status, 15 for PT)
Coordinate services with case managers, families, work personnel, medical personnel, other Ellie staff, and school staff as needed
Attend and participate in required clinical staff meetings and trainings
Participate in clinic marketing, outreach, and networking community events

Requirements

Master’s degree in a behavioral science or related field
5+ years of clinical experience with LCSW-S licensure and supervisor training
Proficient in completing intakes, treatment plans, and clinical case notes
Strong communication skills and the ability to set healthy boundaries
Comfortable working with diverse clients
Familiarity with Office 365 and EHR systems (Valant experience a plus)
Ideally credentialed with insurance panels
