Southwark Council is recruiting two experienced and dedicated Personal Advisors to join their committed Care Leavers Service. This is a rewarding opportunity to make a real difference in the lives of young people as they transition from care into independent adulthood. You will work collaboratively with young people currently in care, as well as those who have left care, providing practical and emotional support to help them achieve positive outcomes. Through planned interventions and consistent engagement, you will empower young adults to develop independence and confidence in their futures. Supporting care leavers is both challenging and highly fulfilling. You will play a key role in helping young people navigate important life areas including education, housing, employment, health, and financial stability.
Job Responsibility:
Work in partnership with young adults, carers, and professionals to assess needs and implement effective support plans
Manage a caseload, prioritising and organising work in a fast-paced environment
Contribute to the development, implementation, and review of Pathway Plans with full participation from young people
Provide clear, analytical written and verbal reports
Attend and contribute to meetings, including safeguarding and court where required
Undertake the statutory role of Personal Advisor in line with relevant legislation and regulations
Build and maintain strong relationships with internal and external agencies
Conduct regular visits with young people, including those living outside the borough or in higher education
Offer guidance and signposting around housing, benefits, education, training, employment, and health
Maintain accurate case records and documentation
Participate in service development, training, and supervision
Requirements:
Experience working with looked after children and/or care leavers
Strong understanding of legislation relating to care leavers
Ability to manage a varied caseload and adapt to changing demands
Excellent communication, organisation, and report-writing skills
A proactive and empathetic approach to supporting young people
Ability to respond to urgent situations and provide timely support
