Job Responsibility:

  • Ensure the people we support receive high-quality, person-centred care
  • Become a trusted guide for colleagues to ensure consistency is followed
  • Help people build independence, develop new skills and make choices that reflect the life they want to live
  • Set meaningful goals and recognise the potential in everyday moments
  • Support colleagues to ensure Support plans are being followed correctly
  • Support with daily routines from personal care and medication to meal preparation, shopping and maintaining a comfortable home
  • Play a key part in coaching and mentoring support workers
  • Share good practice, offer mindful feedback and help create a positive, inclusive team culture
  • Contribute to reviews, support continuous improvement and work in line with Dimensions’ values, policies and regulatory standards
  • Building strong relationships with families, circles of support and colleagues

Requirements:

  • FULL UK/EU Driving Licence is essential
