Job Description:

An established Primary Care Network in Taunton, Somerset is seeking an experienced Lead Clinical Pharmacist to lead and develop its pharmacy team across two GP practices. This senior, patient-facing role requires autonomous practice as an Independent Prescriber and involves medicines optimisation, structured medication reviews, and leadership in medicines safety and governance.

Job Responsibility:

  • Conduct medication reviews for poly-pharmacy, multi-morbidity, care home, and vulnerable patients
  • Act as an Independent Prescriber, implement changes, and monitor high-risk medicines
  • Lead repeat prescribing systems, reconciliation post-discharge, and medicines safety initiatives
  • Supervise and develop Clinical Pharmacists and Pharmacy Technicians
  • Drive audit, quality improvement, QOF, CQC compliance, and service development
  • Provide training and expert advice to MDT colleagues

Requirements:

  • GPhC registered
  • Independent Prescriber
  • MPharm (or equivalent)
  • leadership skills
  • medicines optimisation and governance experience
  • proficient in EMIS

Nice to have:

3+ years advanced primary care experience
