Baptist HealthLead Ambulatory Care Coordinator is hiring a in Jacksonville, Florida. This is a Full Time Day shift opportunity located at our Baptist Metro Square campus. Shift Details: Monday - Friday 8pm - 4:30pm

Job Responsibility

  • Serves as the first line of clinical support to the ambulatory care coordination team for day-to-day operations
  • Coordinates training, onboarding and orientation of new team members
  • Assisting and maintaing adequate staffing coverage
  • Serves as clinical resource for the implementation of new services

Requirements

  • Bachelor's Degree - Nursing
  • 3-5 Years Medical Surgical Experience Required
  • 3-5 Years Case Management Experience Required
  • Basic Life Support (BLS) Required
  • Licensed Registered Nurse Required

Nice to have

  • Master's Degree - Nursing
  • Certified Case Manager, CM (from ABOHN) Preferred
