Position Summary/ Department Summary: The Associate International Case Coordinator is responsible for supporting the daily operations of the International Patient Center. Working in conjunction with the ICC Team, the ICC Associate will be accountable for delivering on case coordination functions in assigned areas of responsibility which include receiving and processing a variety of requests for information, referrals, forms, as well as triaging orders, notices, and other related information.
Job Responsibility
Process referrals for continuing care services including but not limited to, durable medical equipment, patient transfer or requests for extended care facility beds to the appropriate agency
Supports the coordination of the patient intake process including, but not limited to, reviewing, screening, verifying, and recording information regarding the patient from admission to completion of care in accordance with institutional policies and procedures
Coordinates the case management activities within the assigned unit/team
Functions as liaison with patients, internal teams, and external departments in ensuring appropriate referrals are in place for continuing care
Communicates with patients and/or family members as necessary to assist with requests for information, forms and other related data
Requirements
High School diploma/GED is required
Bachelor’s Degree is preferred
Minimum One year of experience working in health care, customer or language services, hospitality is required
Second language strongly preferred
International life experience and experience serving an international clientele in a work environment
