Job Description:

Hello Neighbor is seeking a Preferred Communities Intensive Case Manager as a key member of the Family Health Services team supporting the successful stabilization, self-sufficiency, and integration of highly vulnerable refugees and ORR-eligible immigrants. The Preferred Communities Intensive Case Manager is responsible for providing a continuum of care through culturally and linguistically appropriate case management services to refugees, SIV’s, and Afghan parolees after their 90-day resettlement period and who possess at least one or more of the following vulnerabilities: minors (under 18), young adults without parents, single-parent households, elderly, LGBTQ+, HIV positive, social/psychological disorders, substance dependency, disability or medical condition, secondary migrant, and survivors of torture/sexual gender based violence. The case management goals are individualized in nature and span across various aspects of individual, familial, and/or household self-sufficiency. This includes but is not limited to physical healthcare navigation, mental health resource attainment, school-related issues, and others. This position aims to address issues with a lens of trauma-informed care and focuses on assisting clients in attaining stability and self-sufficiency. This position will include working with other Hello Neighbor programs as well as external facilitation of care coordination teams.

Job Responsibility:

  • Conduct intakes, assessments, and develop comprehensive, self-sufficiency plans for clients in accordance with Preferred Communities guidelines
  • Provide direct and indirect intensive case management services based on individualized service plans
  • Support clients with complex and high-acuity needs through ongoing engagement, follow-up, and stabilization support
  • Assist clients in achieving self-sufficiency goals as soon as appropriate, while accounting for identified vulnerabilities
  • Apply trauma-informed, culturally responsive practices across all aspects of service delivery
  • Provide extended cultural orientation across service domains as needed
  • Participate in Office Hours on Wednesdays on an alternating schedule with other team members from the Family Health Services department
  • Facilitate care coordination teams as needed to assist clients in navigating healthcare and social service systems
  • Initiate referrals to necessary community-based services as client needs are identified
  • Advocate for the appropriate and consistent use of interpretation services across internal and external service providers, including medical and social service systems
  • Assist with crisis situations as they arise, in collaboration with the program team and supervisor
  • Provide transportation resource navigation, including access to the Medical Assistance Transportation Program (MATP) and conduct bus trainings
  • Support appointment coordination and follow-up to reduce barriers to care and service access
  • Adhere to Hello Neighbor’s confidentiality guidelines and ensure appropriate handling of sensitive information
  • Participate in regular supervision meetings and relevant staff meetings

Requirements:

  • Master’s degree in social work, public health, human services, or a related field
  • Four (4) or more years of relevant and/or applicable experience, with demonstrated ability to effectively work with refugee and immigrant communities
  • Willingness and ability to work in a fast-paced, multicultural environment
  • Valid driver’s license, access to a personal vehicle, and proof of insurance
  • Personal auto insurance with a minimum $300,000 single combined limit of liability coverage
  • Alignment with and support of Hello Neighbor’s mission and core values
  • Flexibility to work occasional evenings and weekends as needed
  • Bilingual or multilingual skills preferred, particularly in priority languages (e.g., Dari, Pashto, Arabic, Swahili, Rohingya, Burmese, Ukrainian, French)
  • Demonstrated ability to appropriately utilize interpretation services
  • Digital literacy and the ability to learn and navigate new software systems
  • Knowledge of social service and healthcare systems
  • medical or behavioral health experience is a significant plus
  • Ability and willingness to participate in rotating on-call responsibilities as required by program needs

  • Clearances required: Pennsylvania Child Abuse History Clearance, Pennsylvania State Police Criminal History Clearance, Federal Bureau of Investigations (FBI) Criminal History Clearance (PA Act 23 & Act 24)
  • Mandatory Reporter and/or PSEA training may also be required depending on job requirements
