Intensive Case Management Coordinator

• Utilize critical thinking and professional judgment to support case management process
• Facilitate and maintain improved healthcare outcomes for members
• Provide advocacy, collaboration coordination, support and education for members
• Evaluate members through care management assessments and information/data review
• Identify high risk factors and service needs
• Coordinate and implement Wellness care plan activities
• Monitor member care needs
• Consult with case managers, supervisors, Medical Directors
• Identify and escalate quality of care issues
• Utilize negotiation skills to secure appropriate options and services
• Provide coaching and support to empower members

• Minimum of 6 months experience in Foster Care and juvenile justice systems, Adoption Assistance, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention services or evidence-based practices applicable to Kentucky SKY populations
• Minimum of 1 year experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling
• Minimum of 1 year experience in Microsoft Office Suite applications including Word and Excel
• Willing and able to travel 80-90% of the time
• Must reside in or adjacent to Jefferson County, KY
• Minimum of Bachelor's degree or non-licensed master level clinician in behavioral health or human services field

2+ years of experience in Foster Care and juvenile justice systems, Adoption Assistance, Behavioral Health Services, Trauma-informed Care, ACEs, Crisis Intervention services or evidence-based practices applicable to Kentucky SKY populations
