We are looking for a dedicated Intake Coordinator to join our team on a contract basis in Burlingame, California. In this role, you will oversee the admission process for new patients, ensuring smooth coordination of care and accurate documentation. This position requires strong organizational skills and the ability to manage multiple responsibilities efficiently while maintaining excellent communication with patients and healthcare professionals. The hours are Sunday-Thursday 8:30AM-5:00PM.
Job Responsibility:
Receive and process patient referrals from physicians and healthcare facilities, initiating the intake process promptly
Coordinate patient care by assigning case managers and clinicians to ensure timely admissions within a 48-hour timeframe
Accurately enter new patient information into the system, verifying all demographic and medical details
Conduct insurance eligibility checks and verify reimbursement availability for services
Communicate effectively with patients and families through follow-up calls, offering clear and supportive guidance
Schedule patient visits in accordance with prescribed discipline and visit frequency requirements
Manage various administrative tasks to support daily workflow and ensure seamless operations
Collaborate with clinical teams to ensure all patient needs are met efficiently and professionally
Requirements:
At least 1 year of experience as an Intake Coordinator in Home Health or Hospice settings
A minimum of 2 years of experience in the healthcare industry with a focus on accuracy and thoroughness
Proven ability to multitask and manage multiple priorities effectively
Proficient in using computer systems, including working with data across multiple screens and software programs
Strong attention to detail and a proactive approach to achieving goals
Familiarity with patient scheduling and record management processes
Knowledge of content management systems (CMS) and production environments
Excellent communication skills and the ability to provide compassionate support to patients and families
