We are looking for an Intake Coordinator to join a fully onsite team in a contract-to-permanent capacity. This position is ideal for someone who enjoys guiding clients through the intake process, maintaining accurate records, and supporting healthcare operations with strong attention to detail. The person in this role will help create a smooth onboarding experience for clients while coordinating closely with internal staff to keep documentation complete and workflows moving efficiently.
Job Responsibility
Manage incoming client referrals and intake requests, ensuring each case is entered accurately and processed in a timely manner
Collect, review, and organize application materials and supporting documentation needed to move clients through the onboarding process
Communicate with clients, families, and internal teams to clarify missing information and provide updates on intake status
Maintain accurate records within intake and healthcare information systems while following established documentation standards
Coordinate with operational staff to support service setup and ensure new cases are routed appropriately
Monitor intake activity, track outstanding items, and follow up consistently to keep files progressing without delay
Provide administrative support related to home health intake functions, helping the team maintain efficient daily operations
Requirements
Prior experience in client intake, admissions, or care coordination within a healthcare-related setting
Working knowledge of healthcare operations and familiarity with documentation processes tied to client onboarding
Experience handling client applications and reviewing records for completeness and accuracy
Comfort using healthcare informatics platforms or similar electronic systems for data entry and case tracking
Background supporting home health agencies or comparable care environments is preferred
Strong communication skills with the ability to interact professionally with clients and internal stakeholders
Excellent organizational skills and a detail-oriented approach to managing multiple active cases
