As an Intake Coordinator RN, you will provide expert clinical assessment and crucial coordination for patients entering our care, ensuring appropriate placement and a seamless transition. Every day you will expertly evaluate patient needs, conduct thorough clinical assessments, communicate with patients and referral sources, and meticulously arrange for suitable services, ensuring timely access to care. To be successful, you will demonstrate outstanding clinical judgment, strong communication skills, and an empathetic, organized demeanor, contributing significantly to optimal patient placement and the continuity of care.
Job Responsibility:
Uses strong clinical knowledge, communication skills, and problem-solving abilities to build productive relationships with physicians and discharge planners at hospitals
Be results focused in order to meet census goals
Meet with referring hospitals every day to help identify and transition patients who will benefit from LTACH services
Perform pre-admission assessments on referred patients
Educate patients, families, hospitals and healthcare professionals on the benefits of LTACH care
Monitor marketing activity through call logs (activity logs) and profiling of customers is a necessary skill.
Requirements:
A minimum of an Associate Degree in Nursing from an accredited nursing program, upon hire and All new-hire Associate Degree Registered Nurses (new graduate and experienced) are required to start an approved BSN program within two years of hire and complete an approved BSN program within five years of hire., upon hire
